Babergh district councillor for Hadleigh North, Sian Dawson, who failed to apologise after lying in a report, could be summoned back to standards board to face further sanctions.
Cllr Dawson was found to have breached three parts of the Suffolk Councillors Code of Conduct when she made untrue claims in a monthly district report to Hadleigh town council last year.
A Code of Conduct complaint against her was upheld by Suffolk's Joint Standards Board sub-committee where she was found to have made false, derogatory, disrespectful and offensive comments.
Councillor Dawson failed to attend the hearing, later claiming she did not know about it and was on leave, although all other participants that received the same information attended.
She was subsequently told she must send a written apology to Babergh vice-chairman and Ganges ward member Derek Davis for the proven breaches.
But Cllr Dawson did not respond as required, failed to contact the monitoring officer with an apology and could be called to explain her actions in front of another disciplinary board, if she is re-elected to Babergh council.
Dawson also faces legal action in a civil court for defamation.
This is not the first time Cllr Dawson has broken the rules while representing Hadleigh as part of the Conservative Group.
In August last year, a planning meeting had to be halted after Cllr Dawson breached planning rules. She was subsequently told she could not carry on being on any panning committee meetings until she had completed the requisite training. To date she has not been part of any subsequent committees.
Before being elected as a Conservative member in 2015, Dawson was forced to resign as a magistrate before she was sacked after being found to have abused her position as a JP.
A panel from the Judicial Conduct Investigations Office looked into allegations that she had twice tried to use her position as a magistrate for personal advantage.
A statement from the JCIO at the time said: "A Disciplinary Panel recommended that Dr Dawson be removed from the judiciary, but she resigned before the disciplinary process had been formally concluded.
"Her resignation took effect from 20 July 2015. Had she not resigned, the Lord Chancellor and the Lord Chief Justice would have removed her from judicial office."
Other incidents include:
Made at least one other false and inaccurate monthly district council report to Hadleigh town councillors.

Picking her nose and eating the contents at a public meeting captured on video.
Being instrumental in a Babergh cabinet reshuffle which resulted in her losing her place along with all her fellow Conservative group members. Consequently the Tories lost all chair and voice chair positions on all other Babergh committees.
Ordered the removal of charity banners in the town.
